2025 Form 1095-C Availability Notice

TransOx, Inc. is utilizing the IRS-permitted alternative furnishing method for Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 calendar year.

Current and former employees who were employed at TransOx, Inc. during the 2025 calendar year and who are eligible to receive a Form 1095-C may request a copy using the secure request form below. Upon receipt of your request, TransOx will furnish the form within 30 days, or by March 2, 2026, whichever is later, in accordance with IRS guidelines.
